Preprinted registration form is available for download below. Please make as many copies as needed, using one form per person, per course. Checks should be made payable to NSU Dental Continuing Education.

Cancellation
Requests for cancellations must be received at least 3 working days prior to the start of a lecture program or one week prior to a hands-on or limited enrollment course. However, for cancellations received after this deadline, 75% of the tuition may be applied toward a future course. Tuitions for no-shows will be forfeited, no exceptions. Please be aware that NSU College of Dental Medicine is not responsible for reimbursement of a non-refundable airline ticket and any other travel expenses in the event that a course is cancelled.
